Rewire Longevity Program Agreement

This Agreement (“Agreement”) is entered into by and between DASH Enterprises, Inc. (“Company” or “we/us”) and the participant (“you”) for participation in the Rewire Longevity Program (“Program”). Please read the following terms carefully before signing.

1. PROGRAM OVERVIEW & COMMITMENT

You agree to fully participate in the Rewire Longevity Program, a 4-month precision health program that includes Brain DNA Testing, Comprehensive Blood Work, GlycanAge Testing, a custom protocol built by Dr. Alok Trivedi, a 30-minute consultation with Dr. Debbie, weekly biometric evaluations, access to a WhatsApp accountability group, and two live teaching and coaching calls with Dr. Trivedi.

You understand that the results you experience depend on your willingness to engage fully, implement your custom protocol, and participate consistently throughout the program. The Company is not responsible for outcomes resulting from incomplete participation.

2. PROGRAM FEE & PAYMENT TERMS

By enrolling, you agree to pay the full Program fee as outlined at the time of registration.

No Refunds: Due to the individualized, high-touch, and data-driven nature of this Program, all payments are non-refundable once made. The testing, protocol development, and personalized resources allocated to your enrollment begin immediately upon payment. Choosing not to participate, withdrawing early, or not completing the Program does not entitle you to a refund or credit of any kind.

If a payment plan has been arranged, all scheduled payments must be completed in full regardless of participation status.

3. HEALTH DISCLAIMER & NATURE OF SERVICES

This section is important. Please read it carefully.

The Rewire Longevity Program is an educational health and wellness program. All content, testing data, protocol recommendations, coaching calls, biometric evaluations, and any other information provided through this Program are for educational and informational purposes only.

This Program does not constitute medical advice, medical treatment, diagnosis, or the practice of medicine in any form. Dr. Alok Trivedi is not acting as your physician, licensed medical doctor, or healthcare provider through this Program. Dr. Debbie’s consultation is provided in a wellness and naturopathic advisory capacity and does not constitute a doctor-patient relationship.

The testing included in this Program, including Brain DNA Testing and GlycanAge Testing, provides data and insights for educational purposes. Results and recommendations based on that data are not intended to diagnose, treat, cure, or prevent any disease or medical condition.

You acknowledge and agree that:

You are solely responsible for any decisions or actions you take based on information, recommendations, or insights shared through this Program.

You will consult your licensed physician or healthcare provider before making any changes to your health regimen, medications, supplements, or treatment plan.

The Company, Dr. Alok Trivedi, Dr. Debbie Stango, and DASH Enterprises, Inc. are not liable for any health outcomes, adverse reactions, or consequences resulting from your participation in this Program or your implementation of any protocol recommendations.

Participation in this Program is entirely at your own risk.

If you are currently under the care of a physician or being treated for any medical condition, it is your responsibility to inform your healthcare provider of your participation in this Program and to obtain their clearance before proceeding.

4. INDIVIDUAL RESULTS & NO GUARANTEE

Results from this Program vary by individual. Factors including your starting health baseline, consistency of participation, lifestyle, genetics, and personal implementation all influence outcomes. The Company makes no guarantees regarding specific health outcomes, biological age reversal, cognitive performance improvements, or any other results.

Testimonials or case examples referenced in marketing materials reflect individual experiences and are not a guarantee that you will achieve the same or similar results.

5. CONDUCT & RIGHT TO REMOVE

The Company reserves the right to remove you from the Program at any time without refund if your conduct is disruptive, disrespectful, or in violation of the terms of this Agreement. We are committed to maintaining a safe and respectful environment for all participants.

6. CONFIDENTIALITY & INTELLECTUAL PROPERTY

All tools, content, protocols, materials, and resources provided through the Rewire Longevity Program are the proprietary intellectual property of DASH Enterprises, Inc. and Dr. Alok Trivedi. They are provided for your personal use only.

You agree not to record, reproduce, distribute, resell, or teach any Program content or materials without prior written permission from the Company.

Your personal health data and participation in the Program will be kept confidential by the Company and will not be shared except as required by law.

7. MEDIA RELEASE

You may be invited to share your experience, results, or testimonials. If you voluntarily agree to do so, you grant DASH Enterprises, Inc. permission to use your name, likeness, voice, and statements in promotional and marketing materials. You may opt out at any time by notifying us in writing at support@drrewire.com.

8. PROGRAM MODIFICATIONS

The Company reserves the right to modify the Program structure, content, delivery format, or schedule as needed to improve the participant experience or respond to unforeseen circumstances. Any material changes will be communicated to active participants.

9. GOVERNING LAW

This Agreement shall be governed by and construed in accordance with the laws of the State of Illinois. Any disputes arising from this Agreement shall be resolved in the jurisdiction of the State of Illinois.

10. ENTIRE AGREEMENT

This Agreement represents the entire understanding between you and DASH Enterprises, Inc. regarding your participation in the Rewire Longevity Program. By signing, you confirm that you have read, understood, and agree to all terms outlined in this Agreement.
