This Agreement (“Agreement”) is entered into by and between Trivedi Enterprises, Inc. (“Company,” “we,” or “us”) and the primary account holder (“you” or “Primary Member”) on behalf of yourself and any family members enrolled under your Family Membership in Rewire Lab (“Program” or “Membership”).

Please review the terms below. By checking the agreement box on the registration form, you confirm that you have read, understood, and agree to all terms of this Agreement on behalf of yourself and all family members you enroll.

Family Membership Definition: For the purposes of this Agreement, Family Membership covers the Primary Account Holder, their spouse or domestic partner residing in the same household, and their dependent children and college students up to age 21 living in the same household. It does not extend to extended family members, relatives, or any individuals not residing in the Primary Account Holder’s immediate household. Enrolled members aged 18–21 are legal adults participating under the family plan and will be required to provide their own acknowledgment of this Agreement.

1. Membership Overview

Rewire Lab is a live group coaching program designed to help individuals and families rewire limiting patterns and build lasting change through structured working sessions, expert coaching, and community accountability.

Your Family Membership includes:

• Access to twice-monthly live group working sessions with Dr. Rewire and authorized coaches for all enrolled adult members (22 and older).
• Access to twice-monthly live breakout sessions led by Ashianna Trivedi for all enrolled children and college-age dependents (up to age 21).
• Access to all recorded adult sessions through the Rewire Lab adults platform for enrolled adults (22 and older).
• Access to all recorded sessions through the Rewire Lab kids platform for enrolled children and college-age dependents (up to age 21).
• Ongoing resources, tools, and materials at the Company’s discretion.
• Community support between sessions.

This Membership remains active on a month-to-month basis as long as payments are current.

2. Primary Account Holder Responsibility

By enrolling in the Family Membership, you are designated as the Primary Account Holder. You are solely responsible for:

• All payments associated with the Family Membership.
• Registering all eligible family members (spouse or domestic partner and dependent children and college students up to age 21 residing in the same household) through the family registration form provided after enrollment.
• Ensuring that all adult family members (18 and older) read and independently acknowledge this Agreement.
• Providing accurate information for all enrolled family members, including names, dates of birth, and contact details.
• Supervising and taking responsibility for the conduct of any enrolled minor (under 18) during Program participation.
• Notifying the Company of any changes to your family enrollment, including additions or removals of family members.

The Primary Account Holder’s agreement binds all enrolled family members to the terms of this Agreement.

3. Enrollment of Children and College-Age Dependents

If you are enrolling one or more children or college-age dependents (up to age 21) in your household, the following terms apply:

• For enrolled members under 18: You represent and warrant that you are the parent or legal guardian of each enrolled minor, that they reside in your household, and that you have full legal authority to enroll them in the Program. You consent on their behalf to their participation in Rewire Lab sessions, including any recording of those sessions.
• For enrolled members aged 18–21: You acknowledge that they are legal adults participating under the family plan. Each enrolled member in this age group will be required to provide their own acknowledgment of this Agreement as part of the family registration process.
• All enrolled members up to age 21 will have access to the Rewire Lab kids platform only. They will not have access to adult sessions or the adults platform.
• You acknowledge that children’s and college-age sessions are age-appropriate working sessions focused on mindset and personal development tools, and do not constitute therapy, counseling, or medical treatment.
• You agree to consult a qualified professional if any enrolled member has medical, psychological, or developmental needs that may be relevant to their participation.
• The Company reserves the right to remove any enrolled member from the Program if their participation is deemed disruptive or inappropriate, without refund.

A separate consent or acknowledgment will be required for each enrolled member under this section as part of the family registration process.

4. Payment & Billing

Monthly Family Membership: $197 USD per month, billed automatically on a recurring basis.

Prepay Options:

• 3-Month Prepay: Pay 3 months upfront and receive 3 complimentary DNA test kits.
• Annual Membership: Pay for 10 months and receive 2 months free, plus 3 complimentary DNA test kits.

DNA test kits will be shipped to the address provided at registration. Kits are non-refundable once shipped.

All payments are non-refundable. Missed sessions, schedule conflicts, or non-use of Membership benefits by any family member do not entitle you to a refund, credit, or cancellation of payments during an active billing period.

5. Cancellation

The Primary Account Holder may cancel the Family Membership at any time by providing 30 days’ written notice to support@drrewire.com. Cancellation takes effect at the end of the current billing cycle and will remove access for all enrolled family members simultaneously.

Prepay members may not cancel mid-term for a refund. If you choose to stop participating before your prepay term ends, no refund or credit will be issued for the remaining period.

Individual family members may not cancel independently. The Family Membership is managed solely by the Primary Account Holder.

6. Non-Transferability

Each family member’s access is personal and non-transferable. Access credentials, session links, and program materials may not be shared with anyone outside the enrolled family unit. Unauthorized sharing may result in immediate termination of the entire Family Membership without refund.

7. No Medical, Financial, or Legal Advice

Rewire Lab provides educational and mindset-based tools for personal growth and performance. It does not provide medical, psychological, financial, or legal advice for adults or minors.

You agree to consult qualified professionals for any needs outside the Program’s scope and acknowledge that all content is for informational and educational purposes only. Participation in this Program does not constitute a therapeutic or clinical relationship for any enrolled family member.

8. DNA Test Kits

DNA test kits provided as part of a prepay option are for informational and wellness purposes only. Results are not a substitute for professional medical advice, diagnosis, or treatment. The Company makes no representations or warranties regarding the accuracy or completeness of DNA test results provided by third-party testing services.

9. Conduct and Right to Remove

To maintain a safe and professional environment for all participants, the Company reserves the right to remove any individual family member or the entire Family Membership from the Program — without refund or credit — for disruptive, disrespectful, or inappropriate behavior by any enrolled member, at our sole discretion.

10. Program Changes

We reserve the right to modify the Program schedule, format, coaches, or content at any time. While we strive to deliver consistent value, specific session times, guest experts, or topics may vary.

In the event the Program is discontinued, members will receive 30 days’ notice and a prorated refund for any unused prepaid period.

11. Media & Recording Use

Program sessions are recorded and made available to active Members through the Rewire Lab platform. By enrolling yourself and your family members, you grant Trivedi Enterprises, Inc. the right to use the likeness, name, voice, or testimonial of any enrolled family member — including minors — in future educational or marketing materials, in any media format, without additional compensation.

If you wish to opt out for yourself or any family member, you must notify support@drrewire.com in writing at least 72 hours before any live session.

12. Legal Terms

This Agreement shall be governed by and construed in accordance with the laws of the State of Illinois. You agree that any dispute arising under this Agreement shall be resolved exclusively in the state or federal courts located in Illinois.

Electronic acceptance of this Agreement through the registration form carries the same legal weight as a handwritten signature under applicable electronic signature laws and serves as consent on behalf of all enrolled family members.

This Agreement constitutes the entire understanding between the parties and supersedes all prior discussions or representations regarding the Program.
